INFORMED CONSENT I acknowledge the Health Counsellor is not a physician and does not prescribe, diagnose, or treat any other physical or mental disorders. No treatment is performed, just education of a new health perspective, as everyone has a different perception of health. The Health Counsellor will not accept responsibility for the way the session is perceived or interpreted. I clearly understand that this counselling/teaching session is not a substitute for medical advice. I obtain advice from my personal medical practitioner for any ailments that I may be experiencing. I acknowledge that no assurance or guarantee is provided to me as to the results of the session. I acknowledge that with varied perception, there can be risks and those risks have been explained to me and I assume those risks. I authorize the Health Counsellor to release or obtain information pertaining to my condition(s) and/or treatment to/from my other caregivers or third party payers. I have read the above noted consent and Ihave had the opportunity to question the contents and my therapy. I confirm my consent to treatment and intend this consent to cover my treatment discussed with me, and such additional treatment as proposed by my Health Counsellor from time to time, to deal with my physical conditions for which I have sought treatment.
